Community HealthCare Connections is made up of ten different services designed to help the uninsured and underinsured individuals in Calhoun County access needed healthcare services. These include the: Nursing Clinic of Battle Creek, Mobile Clinic, Dentists’ Partnership, help with durable medical equipment and medical transportation through the Health Assistance Fund (HAF), help with chronic illness prescription medication and medication co-pays through the Prescription Drug Access Program (PDAP), Lions Club Eyeglass Program, Chiropractic Program, supplemental testing strips and meters through the Diabetic Test Strip Access Program, and Navigation Services including Affordable Care Act education, enrollment, and eligibility determination. Key to our success is the identification and removal of barriers that prevent people from accessing needed health services, including lack of personal identification documents, lack of healthcare education or access to food and hygiene supplies. In addition, these services have proven to reduce the number and cost of patients turning to the Emergency Department for care. The role of Community HealthCare Connections is to help individuals access needed healthcare services, despite the barriers that they may experience. That could involve delivering services directly or coordinating the provision of services with partnering community organizations.
