The resident, along with family members, actively participates in the planning process. Each care plan is individualized, taking into account a resident's likes and dislikes, hobbies, interests and functional capabilities. Our dedicated social service staff serves as both an advocate and advisor to the resident and family members. We are well aware of the difficulty residents and families have when long term care placement becomes a necessity. Social workers make the transition period their top priority, and is assigned to each resident on the day of admission. This person is also available to help family members.
