If I could give 0 stars I would... Do no use Forma Glass for your custom glass orders or anything else for that matter. I have purchased a custom glass cut for a table top and after 4 attempted deliveries (where I have waiting for hours on end) I have yet to receive my order. I highly recommend that you do not use Forma.
