The 1967 opening of Clove Lakes made the dream of founder Nicholas Demisay a reality. Since that time, the Clove Lakes name has been synonymous with excellence. Upon admission, each resident is evaluated by our interdisciplinary team. The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychological and therapeutic needs. The resident, along with family members, actively participates in the planning process. Each care plan is individualized, taking into account a resident's likes and dislikes, hobbies, interests and functional capabilities. Our dedicated social service staff serves as both an advocate and advisor to the resident and family members. We are well aware of the difficulty residents and families have when long term care placement becomes a necessity. Social workers make the transition period their top priority, and is assigned to each resident on the day of admission. This person is also available to help family members.
